This is the bio form for my site if you are a Clan cat:

When you create a cat be Please put this as your Subject!!

Name~Clan, Rank

Name: (if you are a kit, apprentice, or leader, put kit, paw, or star for the last part of your name. If you are a medicine cat, a warrior, an elder, or a queen, put anything you want for the last part. Put anything you want for the first part of your name.)

Age: (remember, moons=months. Make this realistic; don't put three moons if you're an apprentice or eight moons if you're a warrior.)

Gender: (tom or she-cat)

Clan: (AirClan, RainClan, or LeafClan)

Rank: (Kit, Apprentice, Warrior, Queen, Medicine Cat, Deputy, Leader, or Elder)

Physical Description: (Should contain eye color, pelt color, unusual markings, and body size. Preferabely three or more sentences long.)

Personality: (Should be at the very least three sentences long. Describe what they're like, and any gifts they have, like the ability to run very fast or a very strong fighter.)

History: (If your character is a kit, one or two sentences; an apprentice, three or four sentences MINIMUM; and warrior, queen, medicine cat, deputy, leader, or elder, five sentences MINIMUM.)

Code Word: (You can find this somewhere in the Introduction section.)

Age: (keep it realistic, remember moons=months. Don't say your two moons old and living by yourself; that's impossible.)

Territory: (say where your cat lives, like Empty Meadow or Outlands. If they constantly wander, just put "Wanderer" here.)

Physical Description: (Should contain eye color, pelt color, unusual markings, and body size. Preferabely three or more sentences long.)

Personality: (Should be at the very least three sentences long. Describe what they're like, and any gifts they have, like the ability to run very fast or a very strong fighter.)

History: (Should be at the very least four sentences long; I'll make an exception if the cat's under four/five moons.)

This is an example bio, and it's also my bio:

Name: Winterleaf

Age: Winterleaf was almost fourteen moons old when she died.

Gender: she-cat.

Clan: StarClan.

Rank: Leader of StarClan.

Physical Description: Winterleaf is a small, lithe she-cat. Her smooth, unmarked pelt is a soft white. Her eyes are a leaf green color and deep with wisdom. She has a small, star-shaped scar above her left eye.

Personality: Winterleaf is a sweet, gentle cat. She is very friendly to all. Before she died, Winterleaf had many dreams with StarClan. She was a very gifted medicine cat. Now, Winterleaf walks in the dreams of medicine cats and leaders, sharing her wisdom with the three Clans.

History: Winterleaf used to be ThunderClan's medicine cat. Then the wars between the Clans came. Winterleaf was the cat who decided to tell the Clans to move to a new territory. She had received a sign from StarClan to where they should move. It was a very long ways away, and many cats died during the journey, but Winterleaf survived the long journey. When at last they reached the new territory, Winterleaf helped them into their Clans before she died of exhaustion, being the young cat she was. She was the first cat to join the ranks of the new StarClan above the new territory.
